- My Aged Cae number and funding source (if applicable)

Cancellations and Rescheduling
We require at least 24 hours’ notice for cancellations or rescheduling. Failure to provide sufficient notice may result in a cancellation fee of the full session amount.
Late Arrivals
If you are late:
